Brief Summary of Employee Benefits

HEALTH COVERAGE OPTIONS*

Hofstra Exclusive Provider Option (EPO) – Utilizing the Empire BlueCross BlueShield network of healthcare providers and facilities. Care must be received within the Empire BlueCross BlueShield network. Referrals are not required.

Hofstra Point-of –Service (POS) – Also uses the Empire BlueCross BlueShield network, but allows for coverage outside the network as well, after satisfaction of an annual deductible.

Optional Plans:

  • Oxford HMO (Liberty Network)
  • HIP Health Plans HMO and POS

All plans include prescription drug coverage.

VISION and DENTAL COVERAGE

If you enroll in the Hofstra POS plan, in addition to medical coverage, you and your family (if applicable) will also receive vision benefits, through EyeMed Vision Care.

If you enroll in the Hofstra EPO plan, in addition to the medical coverage, you and your family (if applicable) will also receive vision benefits through EyeMed Vision Care, and dental benefits under the Delta Dental Premier Program. This plan allows for coverage within and outside of the Delta Dental Network.

However, if you choose one of the Oxford or HIP options, you may enroll in the Delta Dental plan. This plan is contributory and is only available for individual (employee) coverage -- it does not cover dependents.

PENSION PLAN

The Hofstra University Pension Plan is a tax deferred annuity arrangement (IRS Code Section 403(b) annuity). The plan is voluntary with a one-time irrevocable election to participate which begins after you reach age 26 and you meet the eligibility requirements.

TAX DEFERRED ANNUITY PLAN

In addition to the Pension Plan, Hofstra also offers a voluntary plan for additional pre-tax contributions over the mandatory amounts required for the pension plan. IRS regulations govern the maximum annual contribution you can make to this plan.
