	Course ID: The Course ID is in the navigation path in the upper left corner. In this example, the Course ID is STBB101A; it will be different in your course.

	Information about the instructor
	Instructor's Name:
	Instructor's Blackboard User ID:
	Instructor's e-mail address:
	Instructor's telephone:

	Information about the user to be added
	User's Name:
	Is the user registered at Hofstra? Yes No
	Why is the user being added to the course? If Other please explain:
	User's Blackboard User ID: (not required if not registered at Hofstra)
	User's e-mail address:
	User's telephone:
	Form is being submitted by (full name):
