What camp forms do I need to submit?
After you have registered your child for either camp,
Please follow the directions below to determine which forms you need to submit for your child. (Haven't registered yet? Click here)
- Print and fill out the Medical Form
- This form must be submitted for either camp by May 1
- Why is this important?
- If your child is attending ANY SESSION of Specialty Camp
- Print and fill out the Swim Card
Do not fill these out if your child is attending sports academy camp

- If your child is attending Travel Camp
- Print and fill out the Parental Consent Form for Travel Camp
- If your child is attending any off-campus trips or sport activity

Registering for Camp by Fax, Mail, or In-person
You can register online for Specialty Camp and Sports Academy Camp, but if you prefer to register by Fax, Mail or In-person:

The Medical Form is extremely important and must be submitted before May 1
medical form, medication and camp infirmary Our Camp Infirmary is staffed with full-time RNs. Each camper must have a physical examination within one year prior to his or her first day of camp. The medical forms will be mailed to you after your child’s registration is processed, or you may download the medical form from our Web site. These forms must be completed by your pediatrician and returned to the camp office by May 1. The parents of any child needing medication must meet our RNs with the child’s medication at the Camp Infirmary on Thursday, July 1 from 10 a.m. to 2 p.m. The Camp Infirmary is located in Room 131 of the Physical Fitness Center, North Campus. State law requires a Physician’s Medication Request Form to be on file to allow medication to be administered during the camp day. Special medications for allergies, etc., or specific doctor’s instructions must be hand-delivered to the Hofstra Camp Infirmary prior to your child’s camp attendance. It is not legal to have children carry their own medication!
