Need Help Opening the Forms?

To use the Microsoft Word version on one of these forms:

• In Internet Explorer, right-click on the link (Mac: control-click) and choose "Save Target as ..." (Mac: "Download link to Disk"). Save the file to your computer and then use Word to open the file.
• In Netscape, left-click on the link (Mac: simply click on the link), and choose "Open Using Word" (Mac choose "Open Using Application" and choose Microsoft Word as the application).

To use the PDF version on one of these forms:

• In both Netscape and Explorer (PC and Mac), simply click the link. The PDF file will open in a new browser window. The form can be saved by clicking the "Disk" icon on the Acrobat toolbar at top.
