    Registering for Camp by Fax, Mail, or In-person
    You can register online for Specialty Camp and Sports Academy Camp, but if you prefer to register by Fax, Mail or In-person:


Medical Form Details
The Medical Form is extremely important and must be submitted before May 1

New York State law requires a Physician's Medication Request (on the medical form) to be on file to allow medication to be administered during the camp day. Special medications for allergies, etc., or specific doctor's instructions, must be hand-delivered to the Hofstra Camp Infirmary prior to your child's camp attendance. It is not wise or legal to have children carry their own medication! Every effort should be made to administer medication at home, as it presents a disruption in the camper's day. However, if your physician feels that it is necessary, please submit the completed form before medication is sent to camp. A separate form must be completed for each medication. State law does permit administration of medication during the camp day, provided that there are written directions from the physician and parent. Campers are not at this time allowed to carry medication of any kind on their person, to take medication without official written directive (from the physician and the parent), or to take medication without supervision.
