Student Club Forms

• Single Engagement Contract-Use this form to contract artists/performers on campus. The service rendered may be paid or unpaid.
• Guest Lecture Contract-Use this form to contract guest speakers who are receiving payment for their speaking engagements on campus.
• Guest Lecture Volunteer-Use this packet if you are contracting a speaker who does not require payment for their services.
• Musician/Accompanist Contract-Use this form to contract artists/musicians/performers. They may be paid or unpaid.
• Photographer/Videographer Contract-Use this packet to contract photographers/videographers who record or take photographs of events on campus.
• Conferences/Hotels/Travel Form- Use this packet to pay  for  conferences, hotel reservations or for travel and mileage reimbursements.
• Lackman Food Services packet-Use this packet  to purchase food from Lackmann Culinary Services
• Outside Food Vendor packet- Use this packet to purchase food from an outside vendor.
• Reimbursement Form-Use this packet to get reimbursed for items purchased through personal funds. Only items covered under SGA policy will be reimbursed by SGA funds. Items not covered under SGA policy can only be reimbursed with club income (non SGA funds).
• Budget Transfer Form-Use this form to transfer funds from your club’s budget to another club or department on campus to cosponsor programs.
• Donations Form-Use this packet if you wish to donate funds from club income to an outside organization.
