Lost Key Procedure
All lost keys MUST be reported to Public Safety prior to requesting a replacement key(s). All replacement key requests must be made by using the Hofstra University Key Request Form. If you are requesting a new key, go to the Key Request Procedure Page.
As a request for a replacement key, make sure to properly mark the "ARE THESE REPLACEMENTS FOR LOST OR STOLEN KEYS" answer as "YES" and indicate the Public Safety Report Number in the proper location on the form.
