Dependent Federal Verification Form

Use this form if you are a dependent student and have been notified that you must complete the verification process for the school year. Please read the form carefully and return it, with any required supplemental documentation, to the Office of Financial Aid.


Dependent - Request for Re-Evaluation

Use this form to request a reconsideration of your application for financial aid for the academic year if you are a dependent student. Return the completed form, and supplemental documentation, to the Office of Financial Aid.


Independent Federal Verification Form

Use this form if you are an independent student and have been notified that you must complete the verification process for the school year. Please read the form carefully and return it, with any required supplemental documentation, to the Office of Financial Aid.

Aid For Part-Time Study (A.P.T.S)

To apply for Aid For Part-Time Study (A.P.T.S), complete the online TAP application (www.hesc.ny.gov ), submit your (and your parents if applicable) signed New York State Tax Returns and W2s, read and complete the APTS application and submit to the Student Financial Services Suite, Memorial Hall Rm 206.


GRE® Fee Reduction Program

Use this form to request a GRE® Fee Reduction Certificate. These certificates are available for college seniors and unenrolled college graduates who meet eligibility requirements. A Fee Reduction Certificate may be used for one GRE General Test and/or one GRE Subject Test. Eligibility for participation in the Fee Reduction Program is limited to one time only. Individuals meeting the eligibility requirements will be required to pay 50 percent of the full test fee. For more information about the GRE® test please visit http://www.ets.org/gre/

Private Student Loan Self-Certification Form

The Federal Government mandates that private student loan lenders require borrowers to complete a Self-Certification form when applying for a private student loan. You must sign the form and return to your lender. Always check to see if you can E-Sign the form on your lender’s web site prior to completing this form.


Title IV Authorization for Students 

Students who receive federal aid (Pell, SEOG, Stafford Loans, Perkins Loans, Grad PLUS) must complete the online form to have these funds applied to any charges other than current term tuition, fees and room and board. For instructions on how to complete this authorization on the Portal, please visit www.hofstra.edu/fundauthorization.


Title IV Authorization for Parents

Parent recipients of the PLUS loans must complete this form to have these funds applied to any charges other than current term tuition, fees and room and board. Complete and return to the Office of Student Financial Services in Memorial Hall Room 206.


Spring Loan Request

If you would like to apply for loans to cover your spring balance, please complete this form after you have registered and submit it to Student Financial Services located at 206 Memorial Hall. This only applies to students who received a summer/fall Stafford, PLUS/GPLUS or private student loan.


Winter Loan Request

To apply for a winter loan, please complete the form and submit it to Student Financial Services located at 206 Memorial Hall after you have registered for the term. The form can be found by clicking on Winter Loan Request above.  Please note: Students are only eligible for private loans during the winter session!  You must have an approved loan application before your form can be processed.


Summer Loan Request

If you would like to apply for loans to cover your summer balance, please complete this form after you have registered and submit it to Student Financial Services located at 206 Memorial Hall.
