Dependent Verification Worksheet

Use this form if you are a dependent student and have been notified that you must complete the verification process for the school year. Please read the form carefully and return it, with any required supplemental documentation, to the Office of Financial Aid.


Dependent - Request for Re-Evaluation

Use this form to request a reconsideration of your application for financial aid for the academic year if you are a dependent student. Return the completed form, and supplemental documentation, to the Office of Financial Aid.


Independent Verification Worksheet

Use this form if you are an independent student and have been notified that you must complete the verification process for the school year. Please read the form carefully and return it, with any required supplemental documentation, to the Office of Financial Aid.


Independent - Request for Re-Evaluation

Use this form to request a reconsideration of your application for financial aid for the academic year if you are an independent student. Return the completed form, and supplemental documentation, to the Office of Financial Aid.


Aid For Part-Time Study (A.P.T.S)

To apply for Aid For Part-Time Study (A.P.T.S), complete the online 2012-13 TAP application (www.hesc.ny.gov ), submit your (and your parents if applicable) signed 2011 New York State Tax Returns and W2s, read and complete the APTS application and submit to the Student Financial Services Suite, Memorial Hall Rm 206.


GRE® Fee Reduction Program

Use this form to request a GRE® Fee Reduction Certificate. These certificates are available for college seniors and unenrolled college graduates who meet eligibility requirements. A Fee Reduction Certificate may be used for one GRE General Test and/or one GRE Subject Test. Eligibility for participation in the Fee Reduction Program is limited to one time only. Individuals meeting the eligibility requirements will be required to pay 50 percent of the full test fee. For more information about the GRE® test please visit http://www.ets.org/gre/

Federal Perkins Loan Master Promissory Note

If you have been awarded a Perkins Loan, this is the Promissory Note that you must complete.


Private Student Loan Self-Certification Form

The Federal Government mandates that private student loan lenders require borrowers to complete a Self-Certification form when applying for a private student loan. You must sign the form and return to your lender. Always check to see if you can E-Sign the form on your lender’s web site prior to completing this form.


Title IV Authorization for Students 

Students who receive federal aid (Pell, SEOG, Stafford Loans, Perkins Loans, Grad PLUS) must complete the online form to have these funds applied to any charges other than current term tuition, fees and room and board. For instructions on how to complete this authorization on the Portal, please visit www.hofstra.edu/fundauthorization.


Title IV Authorization for Parents

Parent recipients of the PLUS loans must complete this form to have these funds applied to any charges other than current term tuition, fees and room and board. Complete and return to the Office of Student Financial Services in Memorial Hall Room 206.


Title IV Rescindment

Use this form (there is both a version for students and one for parents) should you wish to rescind a previous authorization you have made for the use of federal student aid. Complete and return to the Office of Student Accounts at Hofstra.


Spring Loan Request

  • Spring 2013 Loan Request Form
    If you would like to apply for loans to cover your spring balance, please complete this form after you have registered and submit it to Student Financial Services located at 206 Memorial Hall. This only applies to students who received a summer/fall Stafford, PLUS/GPLUS or private student loan.

Summer Loan Request

Please complete the appropriate form below and submit it to Student Financial Services located at 206 Memorial Hall after you have filed the 2013-2014 FAFSA

IMPORTANT!! PLUS loans for summer 2013 cannot be completed on studentloans.gov until April 1, 2013.

  • Summer 2013 Loan Request Form
    Use this form if you are NOT graduating in August 2013 or December 2013 or if you are enrolled in Summer Session I and another Summer Session.
  • Summer/Fall 2013 Loan Request Form
    Use this form ONLY if you are graduating in August 2013 or December 2013 or if you are ONLY enrolled in the First Summer Session (SSI) which begins on May 20, 2013. 

PLUS Loan Application

To apply for a parent PLUS loan please visit www.studentloans.gov . Under "Manage My Direct Loan," click on "Sign In" to complete your PLUS application. Note: You will need a federal PIN to complete this process. For PIN information, visit www.pin.ed.gov . For assistance, please call 1-800-557-7394.


Grad PLUS Loan Application

To apply for a Graduate PLUS loan please visit www.studentloans.gov . Under "Manage My Direct Loan," click on "Sign In" to complete your Graduate PLUS application. Note: You will need a federal PIN to complete this process. For PIN information, visit www.pin.ed.gov . For assistance, please call 1-800-557-7394.


GPLUS/PLUS Loan Increase Request Form

To request an increase in your Graduate PLUS or Parent PLUS loan, please complete and submit this form to Student Financial Services located at 206 Memorial Hall.
