Request Forms For Current Students

Requesting Field/Mobile Equipment
Need field/mobile equipment? Please fill out the corresponding form(s) and email them to er@hofstra.edu. For any questions, call the LHSC Equipment Room at 516-463-4906.

Current Herbert School students can now request the use of the LHSC van, known as the “Herbert Hauler.” There are two important steps in the process:

  1. Become an “approved” driver: This involves a successful background/DMV check and completion of an online driver safety course.
  2. Request the van: This involves getting approval of a request for the van, including dates, times, and purpose, and approval for any passengers.

This process will take time, so to ensure your request is processed in a timely manner, please make sure to complete the required forms several weeks in advance and submit to Fred O’Neill (Frederick.ONeill@hofstra.edu), who can also answer any questions.
