Simply choose the form that you need, print it out, and complete it. Please follow the directions for each form as they are all different. Dependent Federal Verification Form
Use this form if you are a dependent student and have been notified that you must complete the verification process for the school year.
Dependent - Request for Re-Evaluation
Use this form to request a reconsideration of your application for financial aid for the academic year if you are a dependent student. Return the completed form, and supplemental documentation, to the Office of Financial Aid.
Independent Federal Verification Form
Use this form if you are an independent student and have been notified that you must complete the verification process for the school year.
Independent - Request for Re-Evaluation
Use this form to request a reconsideration of your application for financial aid for the academic year if you are an independent student. Return the completed form, and supplemental documentation, to the Office of Financial Aid.
Aid For Part-Time Study (A.P.T.S)
To apply for Aid For Part-Time Study (A.P.T.S), complete the online TAP application (www.hesc.ny.gov ), submit your (and your parents if applicable) signed New York State Tax Returns and W2s, read and complete the APTS application and submit to the Student Financial Services Suite, Memorial Hall Rm 206.
ETS provides the fee reduction program for individuals who demonstrate financial need and for national programs that work with underrepresented groups. A Fee Reduction Voucher may be used for one GRE® General Test and/or one Subject Test. Voucher users pay 50 percent of the regular test fee. Private Student Loan Self-Certification Form
The Federal Government mandates that private student loan lenders require borrowers to complete a Self-Certification form when applying for a private student loan. You must sign the form and return to your lender. Always check to see if you can E-Sign the form on your lender’s web site prior to completing this form.
Title IV Authorization for Students
Students who receive federal aid (Pell, SEOG, Stafford Loans, Perkins Loans, Grad PLUS) must complete the online form to have these funds applied to any charges other than current term tuition, fees and room and board. For instructions on how to complete this authorization on the Portal, please visit www.hofstra.edu/fundauthorization.
Parent recipients of the PLUS loans must complete this form to have these funds applied to any charges other than current term tuition, fees and room and board. Complete and return to the Office of Student Financial Services in Memorial Hall Room 206.
Spring Loan Request
If you would like to apply for loans to cover your spring balance, please complete this form after you have registered and submit it to Student Financial Services located at 206 Memorial Hall. This only applies to students who received a summer/fall Stafford, PLUS/GPLUS or private student loan.
To apply for a winter loan, you can complete the form or email email@example.com requesting loan funds for winter. The form can be found by clicking on Winter Loan Request above. The only loans available for the winter session are Parent PLUS for dependent undergraduate students, Graduate PLUS loans for graduate/professional students and private loans. If you are borrowing a private loan, an approved loan application must be on file before submitting the form or emailing firstname.lastname@example.org. If you are borrowing a Graduate or Parent PLUS loan you must be enrolled at least half time during the winter and spring combined. Half-time is 6 credits for undergraduate students and 4.5 credits for graduate students.
Eligible students will be automatically offered federal Direct student loans based on summer 2020 registration. If you have applied for a Parent PLUS loan or a private loan to cover your summer balance, please complete this form after you have registered and upload it via the Hofstra portal.
PLUS Loan Application
To apply for a parent PLUS loan please visit www.studentloans.gov . Under "Manage My Direct Loan," click on "Sign In" to complete your PLUS application. Note: You will need a federal PIN to complete this process. For PIN information, visit www.pin.ed.gov . For assistance, please call 1-800-557-7394.
Grad PLUS Loan Application
To apply for a Graduate PLUS loan please visit www.studentloans.gov . Under "Manage My Direct Loan," click on "Sign In" to complete your Graduate PLUS application. Note: You will need a federal PIN to complete this process. For PIN information, visit www.pin.ed.gov . For assistance, please call 1-800-557-7394.
GPLUS/PLUS Loan Increase Request Form
To request an increase in your Graduate PLUS or Parent PLUS loan, please complete and submit this form to Student Financial Services. You may submit this form on the my.hofstra.edu portal. For instructions visit hofstra.edu/fasteps.
