HOFSTRA UNIVERSITY REPORTING OPTIONS

Hofstra University is committed to maintaining a safe, supportive, and respectful learning environment. The University encourages all community members to report concerns so that appropriate support, resources, and responses may be provided. Please review the options below to determine the reporting pathway that best suits your situation. To access a reporting form, please click on the appropriate heading. 

Hofstra University supports an environment characterized by open and honest communication and encourages members of the community to seek out the appropriate University personnel when they believe policies or standards have been violated. We want you to feel comfortable approaching the appropriate university personnel where you believe a policy or standard has been violated. 

Reporting Pathways

General Incident Report Form

Submit reports related to behavioral concerns, safety issues, or other non-academic matters not addressed by the options below. If you prefer to speak with Public Safety, please call 516-463-6606 or visit the office. 

  • If you are experiencing a roommate conflict or have concerns involving your roommate or residence hall staff, please reach out to your Resident Director. They are available to meet with you, listen to your concerns, and help you navigate the situation. Whether you need support, guidance, or assistance finding a resolution, your Resident Director is here to help and connect you with appropriate resources.

Title IX Report Form

Report incidents involving sexual harassment, sexual assault, dating or domestic violence, stalking, or sex- or gender-based discrimination. Reports will be reviewed in accordance with the Title IX Grievance Policy and the Student Policy Prohibiting Sexual Assault, Domestic Violence, Dating Violence and Stalking policies.  

Hofstra University prohibits all forms of sex discrimination, including, sexual assault, dating violence, domestic violence, and stalking. If you have been the victim or you know someone who has been the victim of sex discrimination, sexual assault, coercion, sexual exploitation, dating violence, domestic violence or stalking, you may report the incident using the reporting links above, or you can contact the Title IX Coordinator for Student Issues at 516-463-5841 or you can email at  studenttitleix@hofstra.edu. For reports against employees, please contact the Chief Human Resources Coordinator, who is the Title IX Coordinator for Employee Matters, and who can be reached at 516-463-6859, humanresources@hofstra.edu or submit a report at hofstra.edu/ethicspoint

For reports of discriminatory harassment, relationship violence, or sexual misconduct, against a faculty member, other University employee, or other nonstudent, consult the procedures set forth in the Harassment Policy, available at hofstra.edu/harassment. Students should report these matters to the Chief Human Resources Coordinator, who is the Title IX Coordinator for Employee Matters, and who can be reached at 516-463-6859. 

If you are an employee reporting on behalf of a student please remember to inform the student that you are not a confidential reporter and if they would like to speak to someone who is, they can contact the Student Counseling Center or Student Health, or the Interfaith Center.

The University is committed to having a campus environment free from all bias, hate crimes, and harassment on the basis of race, color, religion, sex, sexual orientation, gender identity or expression, age, national or ethnic origin, physical or mental disability, marital or veteran status or any other characteristic protected by state or federal laws (characteristics collectively referred to as “Protected Characteristic”). Bias and discriminatory harassment are prohibited in the campus environment, including all academic, athletic, and school-sponsored activities, and the University may be legally required to investigate both informal and formal complaints of discriminatory harassment. Where discrimination and/or harassment is found to have occurred, the University will take appropriate action to prevent recurrence of harassment and to correct its discriminatory effects, if appropriate. Discriminatory harassment is verbal, nonverbal, expressive, or physical conduct that denigrates or shows hostility or aversion to an individual on the basis of that person’s Protected Characteristic and creates an intimidating, hostile, or offensive academic or residential environment or otherwise adversely affects the individual’s academic opportunities or participation in a University program, activity, or benefit. Hofstra’s definition of discriminatory harassment is interpreted and applied consistent with current legal standards, as well as accepted standards of mature behavior and objective reasonableness.

Hazing Report Form 

Hazing refers to any activity expected of someone joining a group, conducted for the purpose of initiation into or affiliation with a group, or expected of a group member to maintain full status in a group, where the activity is abusive, humiliating, degrading, excessive, or contrary to commonly accepted standards of decency, or risks emotional or physical harm. The requirement or expectation to participate in the activity need not be expressly stated. If an individual reasonably believes that they will not be considered a fully participating member of the group or that they will be ostracized for not participating in particular behaviors (for example, alcohol use), then such implied coercion will be considered hazing.

Submitting a Report Anonymously

Anonymous and confidential reports may be submitted through any form used above. 

*Please note that reports may be made anonymously, but this may limit our ability to investigate and take appropriate action in response to the report. 

EthicsPoint supplements the many existing reporting channels across the University. Hofstra University supports an environment of open, honest communications and encourages community members to approach appropriate University personnel in instances where they believe violations of policies or standards have occurred. However, in situations where you prefer to place a report on an anonymous basis or through an outside party, Hofstra has implemented the EthicsPoint hotline.
